What Does a Doula Actually Do? A Guide for Expecting Families

Oct 5
3 min read

You've probably heard the word. Maybe a friend mentioned her doula after her birth, or it came up in a pregnancy group, or you stumbled across it while searching for support and weren't quite sure what you were looking at. The question we hear most from families new to our work is a simple one: what does a doula actually do?

It's a good question. And it deserves a real answer.

A Doula Is Not a Midwife (Here's the Difference)

A midwife is a medical professional. She monitors your vital signs, manages clinical decisions, and delivers your baby. A doula is something different — not a replacement for your medical care team, but a deeply valuable addition to it.

A doula is a trained support person whose entire focus is on you — your comfort, your confidence, your emotional experience, and your ability to make informed decisions throughout pregnancy, birth, and the weeks that follow.

She doesn't manage your labor. She holds your hand through it.

Two Types of Doula Support

Birth Doulas

A birth doula joins you during labor and delivery. She provides continuous, one-on-one support — physical comfort measures like counterpressure and positioning guidance, emotional reassurance when things get intense, and a steady presence that helps you stay grounded in what your body is doing.

Research consistently shows that continuous labor support leads to shorter labors, lower rates of cesarean birth, fewer requests for pain medication, and higher satisfaction with the birth experience overall. But the benefits go beyond statistics. Having someone in the room who is solely focused on supporting you — not charting, not managing a shift, not juggling three other patients — changes the entire emotional tone of the experience.

At Tennessee Family Doulas, we support all birth choices equally. Unmedicated birth, epidural birth, planned cesarean — your choices are honored, never questioned. We're not there to advocate for a particular birth philosophy. We're there to make sure you feel respected and supported on your own terms.

Postpartum Doulas

A postpartum doula meets you on the other side of birth, when you're home and the reality of new parenthood is setting in. Her role is to help you rest, recover, and learn — because those three things are profoundly connected, and all of them are hard to do without support.

Postpartum doula support can look like overnight care so you can actually sleep, daytime support so you can shower and eat a real meal, guidance on infant feeding and soothing, and a calm, knowledgeable presence who can answer your questions without judgment. For families who need more intensive support, extended live-in concierge care is also an option — the kind of comprehensive, hands-on help that lets the whole family settle into parenthood gently, rather than scrambling through it.

What Makes Professional Doula Support Different

You have people who love you — a partner, a mother, a best friend who had a baby two years ago. That love matters enormously. But love is different from experience.

At Tennessee Family Doulas, this is something we come back to often: this may be your first experience with birth and babies, but it isn't ours. Our founder, Tanya Grabbe, has spent over a decade as a ProDoula Lifetime Certified doula, and more than 20 years working in hands-on newborn and family care. That depth of experience — held calmly, offered without agenda — is what makes professional support genuinely different.

We know what normal looks like. We know what it feels like to support a first-time parent at 3am who isn't sure if what they're experiencing is okay. We've been in that room hundreds of times. You haven't. That's not a criticism — it's simply the nature of being new to something. And having someone beside you who isn't new to it changes everything.

Is a Doula Right for Your Family?

If you're expecting a baby and you want to feel genuinely prepared, emotionally supported, and cared for — not just medically managed — a doula is worth considering. If you're heading into the postpartum season and you already sense that rest and recovery are going to be a challenge, a postpartum doula might be one of the most meaningful investments you make before your baby arrives.

Tennessee Family Doulas serves families in Nashville, Franklin, Brentwood, Murfreesboro, Spring Hill, and the surrounding Greater Nashville area. We'd love to answer your questions, talk through your specific situation, and help you figure out what kind of support would serve your family best.
